Membership, Packages, Gift Cards & Appointment Policies

These Terms & Conditions govern memberships, packages, programs, gift cards, and services provided at SHAYANN SCOTT THERAPEUTICS™. By scheduling or purchasing services, you agree to the following policies.

• Patients must provide accurate health history and updated contact information.
• Patients experiencing fever, contagious illness, or active infection should reschedule.
• We reserve the right to reschedule appointments if illness is suspected for the safety of patients and staff.
• Signature Membership: One 60-minute session/month — $99
• Preferred Membership: One 90-minute session/month — $159
• Includes Integrative Massage Sessions
• Additional sessions at member pricing
• 15% off retail products & supplements
• 15% off initial Wellness Evaluation
• Unused sessions roll over while membership remains active
• Sessions become transferable after 6 consecutive months of active membership
• May freeze for 1–3 consecutive months
• Maximum of 2 freezes per calendar year
• Must have at least one payment processed prior to eligibility
• Sessions do not accrue during freeze
Memberships are month-to-month and may be canceled with written notice prior to the next billing cycle.
At SHAYANN SCOTT THERAPEUTICS™, packages and programs are structured care plans — not promotional bundles. They are customized to support specific therapeutic goals and clinical outcomes.
• Each package or program includes an individualized expiration date determined at purchase.
• Timelines are based on recommended treatment frequency and care plan objectives.
• Expiration details are reviewed and outlined prior to purchase.
Packages are designed to be completed within the recommended timeframe to ensure therapeutic effectiveness and continuity of care.
• Unused sessions are non-refundable.
• Extensions may be considered for documented medical or safety-related circumstances.
• Transfers may be reviewed on a case-by-case basis when appropriate.
• No expiration (per state law)

A minimum of 24 hours’ notice is required to avoid penalty.
$75 fee or forfeiture of session credit.
• Arrivals more than 15 minutes late may be considered a late cancel or no-show.
• A brief grace period may be accommodated when schedule permits.
• Otherwise, the remaining appointment time will be honored.
We reserve the right to refuse service for inappropriate behavior or safety concerns.

All payments for services, memberships, packages, and programs are due at time of service or purchase.
At SHAYANN SCOTT THERAPEUTICS™, we reserve time, clinical preparation, and resources specifically for each patient. Therefore:
• Services rendered are non-refundable.
• Packages, programs, and memberships are non-refundable.
• Retail products may be eligible for store credit if unopened and returned within 14 days.
• Medical or safety-related exceptions may be reviewed and may result in clinic credit only.
• All patient health information is protected under HIPAA regulations.
• Personal information will not be shared without written consent, except as required by law.
• Medical services are provided by licensed healthcare professionals in accordance with Arizona law.
• Non-medical wellness services support relaxation, stress reduction, and general wellness.
• Patients are responsible for following medical advice provided by licensed practitioners.
• SHAYANN SCOTT THERAPEUTICS™ is not liable for injury or damages except where required by law.
These Terms & Conditions are governed by the laws of the State of Arizona. Policies may be updated at any time and changes will be posted on this page.
